Dog Name: Pooch

Reason for surrender: Pet health

Relationship: Owned for 5 years was a gift from a friend.

People: Pooch lived with adults and children. Pooch is playful to everyone he meets and loves to play, especially with the children.

Other Animals: Pooch has been around other dogs when on a walk and he will sniff them and walk around them but don't pay much attention.

Personality/Habits: Pooch activity level is high. He likes to play with balls and squeaky toys. Pooch will growl if you try to take food or a toy out of his mouth but will not bite. Pooch is afraid of taking a bath but will tolerate it after a while. When home with Pooch he will only follow around the owner because he is close to her. Overall, Pooch is playful, affectionate, and friendly.

Training: Pooch is mostly indoors. He is house trained and will wait to use the bathroom the next day if he have to go at night. Pooch is never off a leash because owner doesn't know if he will run away. Pooch has never been crate-trained.

Feeding: Pooch only eats table food like chicken and rice.

Behavior upon intake: Pooch was friendly was lying on the floor, allowed all handling.

PE: EENT WNL, PLN WNL, ORAL EXAM MILD DENTAL TARTAR/GINGIVITS, THORACIC AUSC WNL, ABD PALP WNL, M/S WNL, SKIN/HAIR WNL, UROGENITAL -- ULCERATIVE GROWTH AT PENIS -- PREPUCE IS FUSED WITHIN IRREGULAR PENIS TISSUES.  INTACT MALE.

DX:  IMPRESSION SMEAR OF PENILE MASS -- LARGE VARIATION OF CELL SIZE AND SHAPE WITH HIGH LEVELS OF CELLULAR DIVISION AND VACOULIZATION. 

ASSESSEMENT:  TRANSMISSABLE VENEREAL TUMOR HIGHLY SUSPECTED.  GROWTH IS ON VERGE OF CAUSING URETHRAL OBSTRUCTION. 

PROVIDED 0.9ML CONVENIA SQ AT TIME OF EXAM.

PLAN:  RECOMMEND NEW HOPE PLACEMENT ASAP WITH FOLLOWUP CARE AT LOCAL VETERINARIAN.  IF NO PLACEMENT RECOMMEND EHR.
